Utah County Sick Policy

If your student is registered with a health concern at Springville High School, it is required for parents to complete one of the following health care plans. 

Conditions that require Health Care Plans are:

• ASTHMA Action Plan: Asthma requiring an inhaler or any other rescue medication

• Diabetes DMMO Form and Diabetes CGM Form

• Seizure SMMO Form and Seizure Action Plan

• AAEAP Form: Severe Allergy requiring a student to carry an EpiPen

• Headache/Migrane IHP Form

• Medication Authorization Form for any medical concern that requires your student to take medication during school hours

• 2021-22 Covid Guidelines
